READ THIS FORM CAREFULLY. By signing this document, you acknowledge that participation in
Fellowship of Christian Athletes (“FCA”) activities involves inherent risks, including serious injury,
disability, or death, even if FCA exercises reasonable care. You voluntarily assume these risks and waive
your right to recover from FCA or related parties for personal injury, death, or property damage arising
from these activities. FCA reserves the right to refuse participation if this form is not signed. FCA also
reserves the right to remove you from participation for unsafe or disruptive behavior.
Assumption of Risk. I acknowledge that I am qualified, in good health, and physically and mentally
capable of participating in this FCA activity, and I expressly warrant my ability to withstand both the
physical and mental demands of the activity. I understand that participation involves inherent risks,
including collisions, falls, overexertion, improper technique, contact with equipment, playing surfaces,
or other participants, fatigue, heat or cold exposure, transportation-related risks, and unknown or
unanticipated risks. I acknowledge that protective equipment such as helmets, pads, and
mouthguards may reduce but cannot eliminate risk. I voluntarily assume all risks, whether known or
unknown, including injury, illness, disability, death, or property damage, and whether arising from the
ordinary negligence of FCA, its staff, volunteers, or others, including negligent emergency response
or rescue operations.
Release of Liability. I release and discharge FCA, its leaders, employees, officers, directors, volunteers,
and agents, as well as the owners, operators, and staff of activity premises or facilities (“Releasees”),
from any claims, demands, or causes of action arising from my participation or presence at FCA
activities, including those caused by ordinary negligence. This release applies to my family, heirs,
representatives, and assigns. I covenant not to sue and forever release Releasees from liability. This
waiver does not cover claims for gross negligence or willful misconduct.
Indemnification and Hold Harmless. I agree to indemnify and hold harmless Releasees from all claims,
liabilities, suits, demands, and expenses, including reasonable attorneys’ fees and insurance-related
costs, arising from my participation in FCA activities, including claims resulting from my own or FCA’s
ordinary negligence.
First Aid and Emergency Medical Treatment. I consent to receive first aid or emergency medical
treatment, including hospitalization or surgery, if deemed necessary by FCA agents or medical
personnel. I agree to pay all associated costs. I release and hold harmless FCA and Releasees for any
claims arising from such treatment or medical services. I acknowledge that I am responsible for
managing my own medical conditions, medications, allergies and dietary restrictions.
Use of Image, Likeness, and Media. I understand that FCA owns all photographs, videos, and other
recordings taken of me during activities. I grant FCA and Releasees the right to use, reproduce,
publish, display, distribute, or sell my name, image, and likeness in any media, including digital, print,
or social media, for ministry promotion, training, publications, advertising, sale, or other purposes. I
also consent to media coverage by local news organizations and to staff, volunteers, or other
participants taking photographs or recordings, with FCA permitted to use or sell such media,
distribute, display, or publish it as FCA deems appropriate.
Agreement & Acknowledgment. This waiver constitutes the complete agreement between the parties
and supersedes all prior agreements or understandings. Modifications may only be made in writing
and signed by all parties. I have read, understand, and voluntarily sign this waiver. I understand that
by signing, I give up substantial rights.
